RN / LPN Job Description - Casual, part-time

About the Role:

The Registered Nurse / Licensed Practical Nurse is responsible for providing exceptional care to patients, while working closely with physicians to ensure optimum clinic operations and patient experiences. RN / LPN will be part of a compassionate team dedicated to welcoming and serving newcomer individuals and families. In this position RN / LPN will be able to work with a dynamic and diverse community. RN / LPN will be able to make a real difference in the lives of newcomers with complex life histories.

Job responsibilities include, but are not limited to:

· Direct patient care: Working with patients to assess learning needs & then develop/implement culturally informed, holistic (physical, mental, emotional & spiritual) care plans within a multidisciplinary team with the expectation of self-management & evaluate whether patient learning needs were met & modify plans as needed.

· Collecting, documenting, reporting & monitoring changes in health status using critical judgment skills

· Patient education by working one on one or in group settings with patients and/or families. Assist with the Indigenous Diabetes Wellness Program & other specialties’ clinics

· Engaging with patients of diverse ethnocultural backgrounds and creating a centre that is welcoming and accepting to difference

· Working with patients’ strengths and gifts to overcome life challenges, such as trauma, to foster their social inclusion

· Initiating forms, supporting and leading an intake process

· Ensuring patient understanding and consent through the use of appropriate language supports such as in-person or over the phone interpreters

· Escorting patients to exam rooms and preparing patients for physicians

· Setting up and assisting with minor surgical procedures, pelvic examinations and IUDs

· Performing diagnostic testing as ordered by the physician including but not limited to urinalysis, simple blood glucose testing, pregnancy testing

· Performing a variety of care interventions and medical procedures within the designated scope of practice

· Calling patients and explaining sample collection, perform teaching and communicate with the clinicians

· Being aware of the health centre’s purpose and values and performing to these values on an ongoing basis

· Accurately charting any medical procedures or interventions

· Following standards of cleanliness and safety

· Scheduling and notifying patients of appointments

· Scanning patient documents as required

· Opening mail, receiving faxes and responding appropriately

· Completing daily, weekly and monthly duties lists

· Participating in the training and orientation of new medical staff and students

· Understanding physician profiles and executing on requests in order to properly support physicians

·  Actively participating in health centre’s initiatives

· Other responsibilities include supporting evidence-based care for chronic disease management, health promotion, screening & other primary health care related issues

· Other duties as required

Qualifications and Skills Required:

· Graduated from an approved Licensed Practical Nurse program, accredited through CLPNA / CRNA

· Current, active registration with the CLPNA / CRNA

· At least one (1) year recent related experience/ Primary Care/community experience

· Completed phlebotomy course or is prepared to do so

· Critical thinking skills and ability to work under pressure

· Experience of working in cross cultural context

· Community Health Centre experience is an asset

· Experience in using electronic medical records and documentation (EMR), Med Access preferred.

· Strong interpersonal, communication and public relation skills

· Experience working in a team setting with other health professionals

· Experience in establishing protocols for program startups, especially developing plans and priorities when workload varies

· Experience in developing plans and priorities with variable workloads

· Experience working in a team setting with other health professionals
